Sometimes the assessment is only the first step: a program, school, or government form needs documentation in a specific format. This category covers that follow-up work, including Disability Tax Credit (CRA form T2201) certification, college and university disability documentation, and admission-exam accommodation requests. I prepare each report to the standard the receiving body asks for, based on the assessment findings. Does this documentation guarantee my application will be approved?
No. I prepare the documentation based on the assessment findings, but the decision on eligibility is made by the relevant body, such as the CRA for the Disability Tax Credit or a college for academic accommodations. No outcome can be guaranteed.
Do I need a completed assessment first?
Usually yes. This documentation is based on assessment findings, so a current and relevant assessment is normally needed. If you are unsure whether an existing report is sufficient, please ask at first contact.
Is this documentation work covered by OHIP?
No. It is a private psychological service. Some extended health benefit plans reimburse part of the fee, so it is worth checking your plan.
How long does the documentation take?
Once any required testing and interviews are complete, a written report is typically delivered about two to three weeks later, allowing time to prepare it to the required standard.
